Financial Aid Forms and Documents

In order for the Financial Aid Office to process your 2014-2015 financial aid award, you must complete and submit various forms and documents. Please print, complete, and submit the appropriate documents to the Financial Aid Office.

Summer 2014 PHEAA Grant Application Now Available! Visit the PHEAA website to apply.  Please note that in order to be eligible, you must be enrolled in both Summer I and Summer II sessions and take a minimum of 6 credits.

 

Financial Aid Applications

2014-2015 New Student Application (PDF) — This application is for undergraduate freshmen and transfer students who will be entering Gwynedd Mercy University during the 2014-2015 academic year.

2014-2015 Returning Student Aid Application (PDF) — This application is for returning undergraduate students who have previously attended Gwynedd Mercy University and will be attending GMercyU again during the 2014-2015 academic year.

2014-2015 Graduate Student Application (PDF) — This application is for all Gwynedd Valley campus graduate students who will be attending Gwynedd Mercy University during the 2014-2015 academic year.

2014-2015 Dependent Verification Worksheet (PDF) — This worksheet is for those dependent students who have been randomly selected to undergo the federal verification process.

2014-2015 Independent Verification Worksheet (PDF) — This worksheet is for those independent students who have been randomly selected to undergo the federal verification process.

2014-2015 Parent Non-Tax Filing Form (PDF) — This document is for those dependent students who have been randomly selected to undergo the federal verification process and whose parents will not file 2013 taxes.

2014-2015 Student Non-Tax Filing Form (PDF) — This document is for those dependent and independent students who have been randomly selected to undergo the federal verification process and will not file 2013 taxes (or whose spouse will not file 2013 taxes).

2014-2015 Alternative Funding (PDF) — This documents indicates a student's options for additional types of financial aid.

2014-2015 Appeal Form (PDF) — This form can be used to appeal to the Financial Aid Office for additional aid based on special circumstances that may not be listed on the 2014-2015 FAFSA .

2014-2015 New Student Checklist (DOC) — This document is a checklist for new and tranfer undergraduate students.  It will help to guide you through the required financial aid processes.

2014-2015 Reduced Income Form - Dependent (DOC) — This document is for dependent students who are reporting a decrease in income in the 2014-2015 year.

2014-2015 Reduced Income Form - Independent (DOC) — This document is for independent students who are reporting a decrease in income in the 2014-2015 year.

FERPA Release Form (PDF) — This release allows students two indicate third parties (i.e. mother, father, spouse, etc.) that the Financial Aid and Billing Offices are allowed to speak to regarding financial information.
